Patient Records Request

Current and former Thornridge patients have the right to request copies of their clinical records. Please review the information below for specific instructions.

How to Submit a Request

All records requests must be submitted in writing. We cannot accept requests by phone or email.

Please mail your written request to:

Your request must include:

  • Your full name and any former names under which you received care.
  • Your date of birth.
  • The approximate dates of service you received.
  • A signed authorization indicating the records you are requesting and any third party you authorize us to release records to.
  • A copy of a government-issued photo ID.

Processing Time

We aim to process records requests within 30 days of receipt. Complex requests or requests involving records from before 2018 may require additional time.

For actual records requests, please submit your request by mail using the process described above. Our Records Department cannot accept requests by phone.

Records from the Pre-2018 Period

Thornridge retains patient records from its pre-2018 inpatient and residential programs in accordance with applicable state retention requirements. Requests for these records are processed through the same mailed-request system described above and may require additional verification given their age and sensitivity.

Fees

A fee may apply for records requests to cover the cost of retrieval, copying, and mailing. Our standard fee schedule is available upon request.
